Possibly, but not likely as it depends on a number of factors.
The receiving phone usually provides a specific ring tone based upon the alert-info or caller-id (CID) that is seen....and this depends on the make and model of phones, Some will only react to alert-info whereas other will do both. This is usually done to help users define and understand the type of call - internal, external, ring group, IVR or other, rather than from who in the event of a transfer.
One issue that arises is what information is passed along in the transfer? In most cases, it is more desirable to see the original number (and possibly the Cname) than it is the CID of the extension that transferred the call. This makes it almost impossible to use the CID as it will change with every call. Perhaps it is not an issue in your case.
The bottom line is that you will need to do some investigative research with a capture to see what info is inserted into the alert-info fields during a call while you transfer. Then you will need to see what may be possible to manipulate the field from your phone. My guess is that you will not have this level of control and that even if you did, there is probably not a way for the phone to know if the call is a new one or a transfer as even then, there are a couple of different ways to transfer. You will need to examine each leg - from you phone to the S-PBX and then from the S-PBX to the phone (do the capture from the PBX).
Once you see what is in the alert-info field from the PBX to the destination extension, then you will need to match this in the receiving extension so that the appropriate ring-tone can be selected. Then, you will need to conduct different types of calls to the destination extension to insure that your call is unique from the others.
Personally, I do not think it possible to do in this scenario, but I guess you won't know for certain till you check.
