MyPBX SOHO not working and can't connect to web UI

Hello. I have a MyPBX SOHO (S/N MA04V3011140179) which has been working without major issues for 5 years.
Suddenly, without any event that we can recall as the root cause, it stopped working and rejects connections from IP Phones and to the admin interface.
 
The four port LEDs are blinking in orange, and the power LED is blinking in green.
The reset button has no effect even when trying to go to factory settings.
 
Any idea on the issue and how to fix it?
